default
Posted on Mon, 27 Oct 2014
Question: I have been diagnosed as SLE patient almost 7 years back. I have taken treatment at PGI XXXXXXX and gone through the MMR process where i have taken Andoxen and wysolone for long. Then I left taking medicine without consulting doctor, I got married and conceived but got miscarriage. Now again i feel the old symptoms of SLE like swelling in body. I contacted a nefrologist and he find the same disease again. What should I do now? Can I live a healthy life again? How long is its treatment? Can I have to take medicine throughout the life. Can I ever deliver a healthy baby?

Brief Answer:
Pregnancy possible if adherent to protocol.

Detailed Answer:
SLE is a systemic disease with involvement of most of the organs in the body. Kidney is the most frequently affected organ. Many of the patients would improve with cyclophosphamide (endoxan) and steroids which are given in a methodical way as per the international protocol. I have many patients who conceived normally after this protocols. The mistake you have done is the violation of protocol and lack of treatment for so many years after the first episode would create more problems. SLE as such is very bad and can be very violent. I feel that you did not understand the disease and also probably not explained about the (bad) nature of the disease and safety of therapy when given " in time" properly and also the successful stories of pregnancy if treated adequately and planned properly.

Successful pregnancy is possible in SLE with lupus nephritis. Only thing it requires a " strategy". What you need to do now is
1. go back to your nephrologist and confirm the severity of disease now
2. renal biopsy
3. Proper " protocol" to which you should be adherent to
4. plan for pregnancy only a year after you achieve " remission" of your disease activities
