Brief Answer:
You may start your period shortly.

Detailed Answer:
Hi,
Welcome to healthcare Magic.
I am Dr Ramadevi Wani. I will be answering your concerns today.
Mirena reduces the flow of menstrual bleeding. By the end of one year some women may even stop menstruating. This is normal. It is due to the local action of of Mirena on lining of uterus.
since you are cramping and have reddish spotting per vagina, this may be the beginning of your period.
The vaginal itching could be due to infection. Apply local clotrimazole cream twice daily. if the itching persists, please consult gynecologist.
I hope this helps. If you have further queries, I will be happy to answer
