What Are The Repercussions Of Prolonged Testosterone Replacement Therapy?

Question: I'm 86 years of age. Lately I have had several episodes with light headiness including near blackouts while playing golf, Also I am experiencing sweating more than usual. I have been on a testosterone replacement therapy for several years but went off about august of this year. I suspect that my T has tanked. Could this be the cause or do you suspect other problems

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Testosterone is known to cause fluid retention and increase blood pressure.
2. Once of the cause, I can think of at present could be due to you getting off testosterone replacement therapy. This might be leading on to hypotension, particularly in relation to posture.
3. Do check your blood pressure in supine posture and after standing immediately. If there is a fall of more than 20mm of Hg systolic, you possibly have postural. However, it needs to be checked immediately.
4. Age-related changes in blood pressure adjustment, particularly due to baroreflex adjustment is another common cause of light-headedness and blackouts due to postural hypotension.
5. Hypotension can activate body mechanisms, which can increase sweating and this in turn can lead onto further hypotension. Even during golf, in heat and frequent changes of posture, hypotension can be dominant.
6. Check your blood pressure and your medications may require reduction. Additionally, more fluids intake and avoiding alcohol (has diuretic action) and slow change of posture, essential.

Do discuss with your doctor regarding the evaluation of postural hypotension and reduction in hypertension medication.
