What Are The Risks And Treatment For High Creatinine Level With Renal Agenesis?
Question: My baby is 2 days old and has a creatinine level of 1.92. He suffers from left renal agenesis. What are the risks/treatment?
He also has a bilirubin level of 14.7 and is being given light therapy. Is this normal?

One kidney agenesis can affect the other kidney which needs to overwork and compensate.
Your baby will need frequent followups initially monthly then once in few months to detect any compromise in the other kidney and early intervention of problems which may happen later in life the commonest expected being hypertension.
Jaundice occurs in almost all babies, some need phototherapy, which shouldn't be a problem.It will settle in a couple of days.

Creatinine indicates renal function
Detailed Answer:
1) High creatinine indicates insufficient renal function. One value is of no use. Needs atleast twice weekly checkup to see if it's rising or falling.
If it is rising, it can cause fluid retention, low urine output, sometimes altered sensorium too.
2) Hypertension, protein loss in urine, fluid retention, acidosis, are some of the other problems which can be picked up by proper physical examination and lab tests.

If there is insufficient renal function, what will be the line of treatment? Will it be fatal?
As for the creatinine level, it was 1.5, 24 hours after birth and 1.92, 48 hours after birth. The hospital has started topping up breast milk with some more feed.
The baby was born after 39 weeks of gestation through cesarean section, if this information is of any use.

The current level is not that alarming.
It might settle down .
If renal function is inadequate, usually dialysis is done.
But with other kidney functioning I hope it compensates well soon.
It may not be fatal if compensation lasts longer till life. Only followups will tell.
