Question: Hello, i had sex with a prostitute (Nigerian) with condom. Due to stress i had did not check if condom broke. I checked it once( sperm was inside) but not careful. What arr the risks of transmission in case the vondom broke?
I decided to start Pep. I am on 3rd day with mild symptoms. Will pep create any liver or kidneys problem?
Finally i would like to ask if i should start pep under the current circumstamves amd if i have to stop it( assesment of risk) thanks

Thanks for posting your query.
Broken condom has risk of HIV transmission, if that individual has HIV infection. Nigeria is a HIV prevalent country. So we cannot ignore your HIV risk.
PEP has to be started before 72 hours after the exposure. Beyond 72 hours there is no role for PEP.
PEP has to be continued for 4 weeks without any interruption. It will not do any harm to your liver and kidney when you are taking the same for just 4 weeks. It has to be continued under the supervision of a HIV physician. He will examine you and put the appropriate PEP as per the needs. If you have any problem already with your liver or kidney, your physician will put you with appropriate PEP. Not to worry.
If you are not able to take PEP, you have to rule out HIV with appropriate test after the window period and then only you can resume your sexual activities. Blood donation and organ donation are to be avoided during PEP and its follow up period or up to you have ruled out HIV infection.

To be honest from that day and on i have anxiety and stress, i cannot even consentrate to work or play with my kid . I doubt myself if condom broke or not, most probably not (95%), but i play this incident in my mind every minute and i cannot be sure 100% if broke or not, since it was dark and i rushed to leave the dirty place and did not take a second look. Maybe my worries derive because of the origin the prostutute comes from.

I have started pep after 24 hours and take 2 pills every day the same time.

The doctor here did not say something else, like when and if i should do additional checks, blood analysis for any possible health problems prior medication or after 2 weeks like i have read on guidelines of UK for example . The only thing he said, is take it and forget the incident, and that when i finish i should make the hiv test another 2 times. Also he said to me, because i was extremely stressed and do not know if he said it because of this, that pep has almost 100% success.

If i follow pep as prescribed, still i have risks cor infection?
Pep fails? If yes do you know the range of failure?
If you were to do assesment whether i should take pep or not, even if i beleive that 95% the condom did not break should you prescribe it for the peace of mind?

Hi,
Welcome back.
I am able to understand your concern. If the condom was intact after the intercourse, then you would have experienced the filled condom whereas with the broken condom we could expect the same. Anyway, it is too late to argue about the same.
You started PEP, that too by 24 hours. It is 99% sure that this will abort the HIV infection, if any.
PEP has a protocol to be followed. I don't where are you from?
Before starting PEP one should have adequate counseling about the importance of ART drugs, Drug adherence, drug toxicities, minor intolerance and about the follow ups to monitor the adherence, side effects and HIV serology. HIV serology has to be done at baseline, 6 weeks, 12 weeks and after 6 months. Only then the individual can be proclaimed that he/she is free from HIV. If the baseline HIV test was reactive there is no role for PEP, only the treatment.
When one was not sure about the condom status and the HIV status of the partner, it is always safe to take PEP. It will assure the safety and relieves unnecessary mental tension.
