What Are The Risks Of Developing Urothelial Carcinoma Post A Cystectomy?
Question: In late 2014 I was diagnosed with Stage 3 bladder cancer (with sarcomatoid like cells), it was removed, along with my prostate. I chose to have a neo-bladder over a stoma, and I had chemotherapy treatment from February to May, 2015. I had a clear scan at the end of XXXXXXX and late September, 2015 - all clear. I had my UA done during a check-up two weeks ago with my Urologist. The cytology report shows Atypical Urothelial cells present suspicious for urothelial carcinoma. The Urologist appears unconcerned, but he originally also thought that the aggressive bladder cancer was probably not going to need treatment. Two Oncologists disagreed with him. These findings I received today were reviewed by a second Pathologist and e concurred with the interpretation. The Urologist had his nurse tell me that he would see me in XXXXXXX for my next scan and said not to worry about these findings. I am having a hard time ignoring the wording and feel I should be more proactive. Your advice?

After cystectomy and neo-bladder, urine cytology is usually monitored, but often it is false -positive. Among all such tests reporting as atypical urothelial cells, only 5-10% would be later confirmed as recurrence.
So, your urologist is correct in one way. But ideally a recurrence should be picked up as early as possible. Hence, getting the scans now and also repeating the urine cytology in 2-3 weeks may be a good idea. You can also conult your oncologists who initially had put you on the right track.
