I can understand your concern. Dicorate ER consists of Di-valproate sodium and the drug is a mood stabiliser drug. Your wife is taking 500 mg per day and as per my opinion these are not sufficient doses. The doses for her age should be around 750-1500 mg per day at least for proper mood stabilisation. Her episodes of shouting could be due to improper control by medicine.
If we consider side effects then as per my opinion the drug is relatively safe even if she is taking the drug since last many years. The drug may cause nausea, vomiting, altered appetite, weight gain, sedation, dizziness etc as side effect. In some patients it can cause reduction of platelets count. It is associated with damage to liver and hepatitis. Get her liver function tests done to rule our any liver damage. Over all the drug is safe and can be taken for many years.
Continue to give her drugs as per prescription. Discuss with her doctor for any change in dose of the drug if needed.
