I understand your concern. I am Dr. Arun Tank, infectious disease specialist answering your concern.

- Can you upload the photo of your blood culture report? Since some species of Staphylococcus are commensal flora of skin and caught during the blood collection.

I am attaching the photos. I have now had fever for almost 4 weeks. The fever pattern has been almost the same for all 4 weeks: below normal when I get up (about 98.0F) rising to a XXXXXXX of 99.4-99.6F by late afternoon and then slowly declining to normal or 99.0F before going to bed.
A different Blood test also showed Typhidot IGM & IGG positive, so my doctor gave me Kefpod AZ BD for 10 days and Nizonide for a week, but the fever pattern has been exactly the same for 10 days even after discontinuing both medicines. Today he has prescribed Cifran OD for 10 days based on the blood culture test (blood sample taken before any antibiotics taken).
I had exactly the same symptoms last year (though the fever range was higher), when this kind of fever lasted for 6 weeks, and again typhidot Igm and Widal were positive. Period was August+2 weeks Sep 2013.
Symptoms both times are the fever pattern as described above, mild tummy upset/loose motions for 3-4 days, abdominal pain 3-4 days, persistent sore throat/mild cold symptoms from day one, persisting thereafter for the entire 4 weeks/six weeks. Also there is a lot of weakness and tiredness right through.
Also, i keep getting an allergic cold almost every month, but with no fever, just weakness and tiredness. Montair LC is quite effective in providing relief, and I am usually on my feet and fine in 3-4 days. Throat is extremely sensitive to cold foodstuffs and drinks, and sour things. These allergic colds (confirmed by ENT as not being infectious colds) always start in the throat.

Brief Answer:
Suggestive of Typhoid fever.

Detailed Answer:
Hello XXXXXXX XXXXXXX

Thank you for your response.

- I reviewed your report. It says Staphylococcus species. But more of it is suggestive of commensal flora or probable contamination.

- Your history is suggestive of Typhoid fever with additional consolidation of positive Typhi dot test and positive widal test.

- I suggest you to do one more blood culture bottle test specially for Salmonella typhi. followed by its anti microbial susceptibility test. Take the most sensitive drug according to report, take it for full course till the infection is cured under your doctors guidance.

Kindly come up of any query you have. I will be happy to answer your query.
Thank you for your contact to health care magic.
Wish you a best health.

Thank you doctor. My doctor started me on a course of Cifran OD yesterday, for 10 days. I have taken 2 doses by now. My fever is again at 99.4F by 2.00 pm
Will the blood culture test show any results? Also, I have already done a 10 day course of Kefpod AZ from 29 Sep-9 Oct, 2014.
I was wondering how come I have these persistent sore throat/cold symptoms:
1. In the last four weeks
2. Chronic condition of susceptibility to colds, as explained in detail in the previous question.
I would be grateful if you could answer on all points..

Brief Answer:
blood culture at fever spike &treatment of allergy

Detailed Answer:
Hello XXXXXXX XXXXXXX

You have to disease in combination so I am answering for each one separately.

Typhoid fever:

You can take the blood culture once you have a peak of fever, and from multiple sites. This will increases the chance of Blood culture positivity even though you have taken the antibiotic.

Salmonella typhi, a causative agent of typhoid fever may reside in the gall bladder and may cause relapse if proper full dose & course antibiotics was not taken.

Allergy:

For allergy your current treatment is good and you can continue with it. Your sore throat and other condition is related with it & has no correlation typhoid fever.

If you have anything else to ask I will be very happy to answer that. We wish you a best health at healthcare magic.

Hello doctor, I understand that the staphyloccus spp maybe contamination from the skin in the blood sample taken for my blood culture test, but is there even asmall likelihood that my repeated throat soreness/colds might be due to some dormant infection due to staphyloccus spp?

Brief Answer:
No possibility of Staphylococcus spp.

Detailed Answer:
Hello XXXXXXX XXXXXXX

-- Typhidot positive test diverts the diagnosis away from the Staphylococcus species.

- The infections which shows cyclic pattern of fever, abdominal upset, and sore throat never caused by Staphylococcus spp. but, it is seen in typhoid infection.

You can come back for further clarification.
Thank you,
Dr Arun Tank,
