What Are The Symptoms Of Swollen Lymph Nodes?
Hello doctor,
My wife developed a wound on her left thigh few days back. We ignored it and then the surrounding area started swelling. Now the swelling has reached till lymph nodes. We went to a gynaecologist cum obstetrician here . She told that ,lot of infection has developed and suggested below medication.
My wife is currently breast feeding our 5 month old baby. So we asked the doctor whether those medications are safe during lactation. She told that its fine.
Now my doubt is,
Based on what I red in the internet the first two tables are moderately safe during lactation and may not pose any significant risk to the baby, Whereas the third one is contra indicated during lactation.(checked in few websites). As per the doctor the third medicine is required for her cure and is safe to take.
So, can you kindly let me know whether the below medication(especially the third one) is safe to take during lactation and my wife can continue breast feeding normally(as per normal routine). Our baby doesnt take powder milk and was vomiting when we tried bottled(powder) milk in the past. Need your valuable inputs.
Medication:
TAXIM -O 200 (ALKEM Health Science)
Composition : Cefaxime IP (as trihidrate) equivalent to Anhydrous Cefixime 200 mg
Dosage : Two tables daily(Morning and Night)
Metrogyl 400 (J B Chemicals and pharmaceuticals
Compostion : Metronidazole IP 400 mg
Dosage : Three tables a day
SERAX - D (MARS THEMAPEUTICS AND CHEMICALS LIMITED)
Composition : Serratiopeptidase IP 10 mg , Diclofenac Potassium BP 50 mg
Dosage : Two tables daily(Morning and Night)

Age : 27
Height : 5 feet 4 inches
Weight : 60 Kgs
Doesn't have any other medical conditions. Our baby was born under weight(2.2 Kgs) and is currently 5.4 Kgs(almost completing 5 months). Other than the low weight she is usually active.

Please find my observations on medicines prescribed to your wife as below.
1. TAXIM O is an antibiotic preparation containing cefixime that will help fight bacterial infection. Cefixime is acceptable to use during breastfeeding.
2. Metrogyl 400mg contains metronidazole and is also another antibiotic given to help treat the infection. This will not cause any serious side effect in the baby due to breast feeding.
3. SERAX D is anti inflammatory and pain relief medication. It contains serratiopeptidase (to heal the tissues faster) and diclofenac (for pain relief). Serratiopeptidase is best avoided during breast feeding as not much research has been done and also it is not an essential medicine for your wife.
(i) It is better not to give SERAX D to your wife keeping in mind that your baby was slightly low weight and presently breast feeding.
(ii) You can give Diclofenac 50 mg or paracetamol 500 mg tablet. A better choice for your wife will be paracetamol (CROCIN 500 mg) for pain relief to be taken after meals in morning and night. This is widely used in patients who are breast feeding their infant.

1. You are right in understanding that SERAX D has been given to reduce pain and inflammation. It is not an absolute essential and can be substituted with diclofenac 50 mg plain.
2. Earlier I thought the pain is more hence advised paracetamol 500 mg. It is true that paracetamol is a good pain reliever with weak anti inflammatory property. If the inflammation is significant then please give her diclofenac 50 mg twice a day after meals.
3. The recommended brands of diclofenac available are DICLONAC 50 mg (manufactured by Lupin Laboratories Ltd), REACTINE 50 mg (manufactured by Cipla Limited).
4. Please check the printed expiry date when purchasing at your local drug store and verify the brand name and contents.
