Question: Hi my father underwent heart check up . His tmt is normal, attached report with this question. However his echo says 50% ef and somexother complications . Doc strongly feels that he has had a heart attack in the past and put him on some blood thinners and cholestrol reducing tablets . He has been a diabetic for 7 or 8 years . His rest of the organs appears normal according to the check up report and as advised by doc. I have attachdd echo and tmt ( tmt he couldnt go beyond stage 2 due to leg pain and breathlesness , he generally walks slow) . Please can you advise if angiogram must be done or if he can quietly cotinue on med doc has prescribed . I wad wondering if Ct angio can be done to evaluate his block rather than angiogram right away
Brief Answer:
Go for Angiogram starightaway
Detailed Answer:
Dear friend,
Welcome to Health Care Magic
Your father is in good hands
The investigations and management are appropriate
Heart attack could be silent / especially in diabetics…
Breathlessness may be a sign of failure / at times angina equivalent.
Reaching the target rate in short time means added anxiety or sympathetic over activity / addition of beta-blocker is beneficial…
The ECHO is suggestive
He needs further investigation
Thallium stress is ideal to test physiology – but he is not able to do TMT well
CT tests the anatomy… if suggestive one needs angio anyway… so in this situation,
GO FOR ANGIOGRAM STRAIGTAWAY / it is the gold standard… / No treatable condition should be missed / this is true especially in Diabetic
Take care
Wishing all well
God bless
