What Are These Long-lasting Complications And Are They Going To Recovery During Time ?
Question: my wife is pregnant and i am reading literature about possible complications Third and fourth degree tear after delivery !
1.question
it is said that 6–8 in 10 women with a third- or fourth-degree tear will have no long-lasting complications after it has been repaired and given time to heal.
what are these long-lasting complications - and are they going to recovery during time ? I hope Yes
2. How often is colostomy after 4th degree tears- i hope rare - as i understand 4th degree have risk of fistula after fourth degree tear ranges from 0.4%–3% so colostomy rate is 0.4- 3 % only ! Logic !!
A Colostomy is usually indicated with large 4th degree tears, especially when the tear
extends above the levator muscles, or where other risk factors for fistula exist
Although rare, the incidence of rectovaginal fistula after fourth degree tear ranges from 0.4%–3%
This is very rare but, when not treated, carries the risk of a rectovaginal fistula and can be diagnosed by anal palpation in the postpartum period
Thank you very much

Brief Answer:
rare complications
Detailed Answer:
Hi,
Thanks for your query.
1. Most common complication of an anal sphincter tear is stool incontinence. Yes, most do recover over time if the time to heal is enough but a few may end up having the problem for life unless corrected. The other complications include anal/rectal prolapse, fistula, stricture etc
2. Yes, you are right. The need for colostomy is rare as 4th-degree tears are rare. But the only definitive management is colostomy which most often is temporary and reanastomsis is planned in many.
Hope, I answered your query.
Please do ask if any doubts.
