Question: i have essential malignant hypertension, and it took about 6 months to bring it only somewhat under control. i have gained 25 pounds in a few months, all in the abdomen. I am short of breath; have tacchycardia; my skin itches, have nausea, am exhausted. Tryglycerides nearly 600; bun 39; gfr 51; uric acid 9; creatinine 1.1, no protein in urine; cpk 170

Hello,
Firstly, if bp is well controlled on losartan Hydrochlorothiazide then you should continue the same. Next, likely you are having atrial fibrillation which is causing tachycardia and along with diastolic dysfunction, it is responsible for shortness of breath. To get relief from shortness of breath, you should request your doctor for a trial of diuretics like Dytor plus. Also, weight loss will help to relieve shortness of breath.
Here, you are already on cartia 300 for heart rate control and if it's under control then should continue the same or else if heart rate is higher side then request your doctor to add Beta blockers like Metoprolol for heart rate control.
If you are having atrial fibrillation as a cause for tachycardia, which can be seen on ecg, then you should also receive anticoagulation like rivaroxaban or apixaban to prevent stroke, so discuss regarding this with your local doctor.
Now, for weight gain, you should get thyroid test done and if weight gain is contributed by fluid retention, then diuretics will help. Also, reduce calorie consumption, have low salt diet, low fat and oily diet. Have light dinner. Try to walk daily atleast 30 min a day. For triglycerides, you are already been started on zetia 10 mg, so you may continue with the same or if there is no adequate response then should be changed to Fenofibrate and statin.
