Brief Answer:
SNRI/TCA will be better choice

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

I read your query and understand your concerns.

Although there are number of options available for treatment of severe anxiety, I will either one of the SNRI or TCA group of medication will be better for geriatric age. In addition severe anxiety often fails to respond to first line medications such as SSRI group of medications.

I must acknowledge that both group of medications have side effects but the psychotherapy is often ineffective for severe anxiety. Only thing which supports use of psychotherapy is adjunctive use in addition to medications.

In my opinion there is need to see a psychiatrist without delay as severe anxiety is often crippling and causes significant impairment in functioning.

I hope this helps you.
Feel free to write back to me if you have more questions.
Thanks and regards.
