What Can Be Done I Am Trying To Fall Asleep My Own Snore Use To Wake Me Up?

Question: Hello,

Issue started 3 months ago when I am trying to fall asleep my own snore use to wake me up. No gasping / choking and the snoring is not very loud too. I used to snore a little but i had recently got my bmi from 30 to 27 loosing 8 kgs ( i do not snore after falling asleep) . However it happens almost 70 percent of the times that i will hear my own snore while trying to fall asleep. Once I am asleep its all good and I can get good sleep.

ENT examination revealed a deviated nasal septum - Mild . Airway graded as mallampati 2 in sitting position with no other abnormalities. I do not have day time sleepiness. But falling asleep is becoming a problem which takes 2 hours almost. ( I get awakened by my own snore) some times even before falling asleep

I had tracked what happened last night using snore apps and XXXXXXX recordings and this is what I observed

1. Mild snoring is observed first 30-40 minutes. The moment I fell asleep snoring started slowly - light snoring like 4-5 times .. the 6th one was a little heavy again and I think this triggered an arousal and I woke up completely.

2. Slept again snoring noticed - Mild for first 40 minutes. It was almost silent for an hour or so then again very mild snoring with 2 hours of silence again.

My understanding is narrowing of airway looks to be happening the first few minutes and those snore vibrations might be correcting the narrowing later. I am not sure. But it looks to be like snoring arousals are being triggered. Never choked / gasped in any of these episodes or had issues with day time sleepiness.

Any help / guidance is appreciated

1. This will be a good time to get a Level 2 Sleep Study done. It will help differentiate between central, peripheral or mixed sleep apnea, and the severity of the condition.

2. The next step, if peripheral or mixed, should be a flexible nasopharyngeal endoscopy (FNPS). This will help determine the level of the airway narrowing. At times a DISE or Drug Induced Sleep Endoscopy is helpful.

I did have a nasal endoscopy sir. Doctor mentioned there were no findings apart from a mild deviated nasal septum.

1. There can be multiple reasons for such a situation, especially with a reasonably good BMI. However, your current description is not a cause for worry as this is very common. It will be much easier to discuss with the above reports.

2. Kindly check if it was a rigid endoscope nasal endoscopy v/s a flexible endoscope nasopharyngoscopy. If there are any images or a report, uploading these will help.
