Question: Sir, Mr. XXXXXXX XXXXXXX is admitted in the hospital for last 1 week. His sodium level had gone down because of which he became momentarily unconscious a week back and had to be admitted to the hospital.He was also detected with atrial trachycardia and COPD. Two times on the date of discharge he got respiratory fluctuations while just moving in the bed and hence he was re-admitted again. I am unable to upload the reports. As per the reports attached there is nothing grossly abnormal. The CT scan brain shows some new changes which may have lead to the current set of symptoms.
The new changes can be because of new functional difference in the heart. As such heart shows old enlargement of ventricle and atrium.
The lungs may have some infection resulting from loss of concsious XXXXXXX or unable to chew or gulp properly following admission or before admission.
