Hi there

Thanks for the query

There are various reasons for patellar malalignment problems such as Genu varum (Bow legs), Genu valgum (Knock knees), Miserable malalignment syndrome (Squinting knee caps), Rickets, Skeletal dysplasias etc.

Ideally you will need standing scanogram X-rays including your pelvis and hips to your ankles to assess the type of deformity that is causing the patellar malalignment to assess the cause. Blood tests for Bone profile like Calcium, PTH, Phosphorus, ALP will help assess presence of metabolic disorders like Rickets and certain skeletal dysplasia.

Congenital Vitamin-D resistant rickets, Skeletal dysplasia, can be seen commonly since birth. There would have been difficulty to achieve adequate height during growth with increased risk of fractures.

Ideally, getting the scanogram, bone health assessment with blood tests can narrow down the cause.

I hope I have answered the query. I will be available to answer your follow up queries.

Thanking you.
