Question: 30-year-old Latina P1 LMP Seventh of February. Having spotting and backache. One small clot (dime sized) This a.m. Spotting began after rough deep sex three days prior. Complains of backache and has a soft abdomen with what appeared to be menstrual cramps. No history of any STD other than herpes she’s never had a history of PID. She prefers a Spanish speaker, please

I understand your concerns.

Following is my reply:

1) Spotting she experienced today is due to episode of deep rough sex.

2) This episode might have lead to small tear in the vaginal wall. This leads to spotting as described above.

3) I am sure this is unrelated to STD, PID or any other problem.

4) This will heal by itself and there is no need to worry.

Seriously, you didn’t listen to the end of the symptoms. Is she having a spontaneous abortion or is she having the next topic pregnancy? The bleeding has gotten worse over the last three days. I myself am a physician of delivered 80 babies but my wife has a difficult time trusting what I’m telling her I expect that you give me an obstetrician’s response

Do serum beta hCG blood test to confirm pregnancy

Thanks for additional details.

1) If you are suspecting either abortion or ectopic pregnancy, do a pregnancy confirmation blood test immediately ( serum beta hCG ).

2) Simple serum beta hCG blood test to confirm pregnancy is most important to assess the possibility of abortion or ectopic pregnancy.

3) If there is positive pregnancy as per serum beta hCG, an ultrasound scan will help to identify if there is pregnancy and presence of gestational sac (intrauterine or extrauterine).

4) If the bleeding has lasted for 3 days, it is unlikely to be ectopic and could be regular menses. Doing a pregnancy test confirmation will help us to put all the worries away.

5) Let me know report of pregnancy test to proceed further.
