Question: My aunt(39years) already suffers from Low BP, gas trouble and hyperthyroidism. last night she went unconscious, before she lost her conscious she said she had a chest and back pain and feel down. After we bought her back to conscious she was having a mild chest pain. however this has happened 2-3 times. we have experienced this last year and now. Please advice.

Hello,
This is likely be a cardiac arrhythmia like psvt or atrial fibrillation, in which heart rate suddenly goes up and also suddenly decreases. Blood pressure falls during these episodes and one has chest pain, palpitations, dizziness, fainting, blackouts, syncope. This happens due to fall in blood pressure and decreased blood supply to brain.. Hyperthyroidism frequently causes atrial fibrillation so it becomes likely possibility in her.
Usually ecg is normal as it is usually done in between the episodes. ECG done during episodes will show abnormality. Anyway she should get one baseline ecg done as it can sometimes show some abnormality. She will need 24 hour holter monitoring which is ecg recorded for 24 hours. So visit nearby cardiologist for these investigation. Get her TSH test done now.
ln order to abort such episodes you should try to cough hard or pour cold water over the face. She should immediately lie down. She should also drink plenty of water and juices in order to keep her bp up. She should avoid traveling or going out alone.
Till the time her evaluation is complete, you can ask your local doctor for tab Metoprolol Xr 25 mg once a day depending upon the bp readings. This will prevent the heart rate from going up.
If none of the above is successful then last option is of electrophysiological studies which is invasive test which can diagnose and treat the arrhythmia permanently in most of the cases.
