Posted on Mon, 15 Sep 2014
Question: Hi, my dad was diagnosed with hyperthyroidism about 12 years ago and he has been under continuous medication since then (once in 4/5 months routine check up and take the right dose of thyronorm). But of late, he has been complaining of dizziness, sweating, muscle ache and lethargic. Are these symptoms related to hyperthyroidism? Or is there any other diagnosis that we should go about? Thanks in advance.

Brief Answer:
Investigations suggested

Detailed Answer:
Hi
I am Dr Mittal
I would like some clarification-
Does he have hyperthyroidism or hypothyroidism? Thyronorm is usually given for hypothyroidism (unless he had surgical removal of thyroid).

Also, the symptoms could be related to thyroid disorder. I would suggest retesting the thyroid profile. Similar symptoms also happen in vitamin D deficieny- so have that tested as well. And an ekg and dobutamine stress echo would be a very good idea in view of dizziness and sweating- to rule out cardiac condition.

best of luck

Thank doc for your reply. I have 2 reports of TSH test - one of May2013 showing 13.01uIU/ml and another of Aug2013 with 0.2. Can this fluctuate in this range and can you please explain if it's hypo or hyper?
Also we have done ECG and Hitler test in Apollo hospital XXXXXXX in Aug2013 another was nothing to worry according to the cardiologist.

Please advice. Thanks.

Brief Answer:
It is hypothyroidism

Detailed Answer:
It is hypothyroidism
What happens is that after the supplementation of thyroid hormone(thyroxine), the TSH levels come down.
Basically TSH is a hormone that tells the thyroid gland to secrete hormone.
When the thyroid hormones are less, this decreases.
When thyroid hormones are present it becomes less.

I will suggest reduction in thyroid medication dose.
It may also cause significant symptoms

Best of luck

Thanks for your advice. Lastly, I would like to know if hypothyroidism requires dietary control - start, stop and continue? As I have heard it is good to avoid salt, cabbage and carbohydrates. Please help. Thanks

Brief Answer:
About diet

Detailed Answer:
You are absolutely right
Brassica family should be avoided- cabbage, cauliflower- avoid.
Salt need not be restricted if there is no swelling.

Continue medicines, but dose reduction is advisable at that level of TSH

Best of luck
Glad I could be of help
