I have read your query and here is my advise.
* As per my clinical experience, the small diameter of the stool depends upon the lumen
counter of the large colon and external anal sphincter.
* In the instances where there is narrowing of the lumen diameter as stricture, tumor or
other issue ; stenosis or tightening of the external anal sphincter also gives the small
diameter stool.
However, the issue has to be co related with symptoms, clinical findings and supportive evidences of CT scan, colonoscopy and so forth to evaluate it precisely.

There has to be an answer for this weird problem.
per rectal examination is needed for anal sphincter tone evaluation.

Thanks for the follow up.
As the colonoscopy was fine, means the issue with intestine diameter is ruled out.
The withholding of the stool at rectum is most likely due to possible spasm of the anal sphincter, which has to be examined clinically by consultant surgeon through simple per rectal examination ( because daily enema is not the definite cure of the issue ).
