Since a thorough workup has already been done, systemic causes should already have been evaluated for and you need not worry about them. However, muscle strain is unlikely to lead to such a condition.
The next line of workup would be to get a FNAC (fine needle aspiration cytology) followed by biopsy done. However, I doubt if those would be necessary.
