What Causes PSA Elevation In The Blood?

Question: My PSA is 5.5 and was advised for Ultrasound KUB. The report says, both the kidneys are normal in size & cortical echogenicity.No evidence of focal lesion, hydronephrosis or caculus seen. The urinary bladder is normal in appearance.Pre void urinary bladder volume is 270 cc.PVR is insignificant. The prostrate is enlarged measures 38.2X52.1X45.1mm,volume 47 gms as seen on per abdominal examination. The prostatic capsule appears intact. On seeing this report my physician has advised me to take Silodal-D ( 4mg) once in the night.
Please advise.

Hi,
You have not mentioned if you are symptomatic due to the enlarged prostate. This can cause an elevation of PSA as might a urinary tract infection.
A large benign growth of prostate can also cause a PSA elevation, and hopefully that is what is happening on your case.
However, the primary reason why we ask for PSA is to find out an elevation suggestive of malignancy and in your case, at your age, we need to ensure we are not dealing with one, and a transabdominal ultrasound is not adequate for the same, even if they mention the capsule is intact.
Silodal-d contains dutasteride which might mask PSA elevation and I suggest you avoid it at present.
Instead, you can get a total and free PSA test at the best possible diagnostic centre. And if the PSA is high, and free PSA is low, you should have a transrectal ultrasound of prostate and if required a biopsy.
This is meant to be aggressive and I would not be suggesting this course had you been 74.
