I have gone through your query in detail and I have also reviewed the images.
I can see dry, scaly, patches at various body sites.
These scaly patches seem eczematous in origin.
Lack of itching as a prominent symptom is probably because you are already using hydrocortisone cream topically.
However, hydrocortisone won't resolve these patches because it is a very mild topical steroid.
If I was the treating doctor I would have started you on a potent topical steroid e. g clobetasol propionate 0.05% cream, twice daily.
in addition I would have started you on an OTC oral antihistamine e. g cetrizine 10 mg once daily.
Together these would produce a noticeable improvement within 2weeks.
I would like to know more about the lump on buttocks
- Is it painful? How big it is?
- can you upload an image of the lump?

It seems more like a raised patch rather than a lump and I feel that it is part of the same disease process i. e Eczematous Dermatitis.
