I am sorry to hear that you are having issues with your tongue. You describe a white spot that is painful periodically. My first concern is that this could be an early sign of a cancer. This is called leukoplakia and you need to see an ENT specialist to evaluate the lesion. They will take a biopsy of the lesion and then identify the lesion and purpose treatment. It may also be thrush which is a fungal infection of the tongue. This can be treated with oral antifungal medication but you would need to see your medical provider to get the prescription.
