I have reviewed the attached photograph and it seems to be a furuncle.
Is it painful?
What is the duration of the bump?
In such cases I recommend my patients to take oral as well as topical antibiotics.
I usually recommend them to take tablet Cefixime 200 mg twice daily for 5 days along with Mupirocin cream on the affected area twice daily for 5-7 days till it resolves.
These are prescription medicines and you need to consult the doctor in person to get them.

Can be granulomatous reaction to previous insect bite or folliculitis

If it is not painful then it can be a granulomatous reaction to a previous insect bite or folliculitis which has gone unnoticed.
If I were your treating doctor I would recommend you to apply a steroid antibiotic combination for 7- 10 days.
It should start showing signs of remission, otherwise consult a dermatologist and get it examined clinically.
