What Causes Abdominal Pain When Diagnosed With Aortic Dissection?
Question: Sir, Hi one of my friend is suffering pain in the abdomen due to small portion of the intestine getting gangrene. According to family Doctor he has Aortic dissection which is causing problem in the blood flow in the SMA area causing severe stomach pain and Graft has to be inserted in the abdominal Aorta for proper blood flow.
So which doctor we need to consult first Gastroenterologist OR vascular surgeon kindly advise.

After going through the medical details provided by you I would like to tell you that as the basic problem is aorta so it has to be addressed by a vascular surgeon.
In my opinion you should consult and take an opinion from a cardiologist also after consulting with a vascular surgeon.

Thanks for your quick response.
In the meantime we consulted Gastroenterologist due to severe abdominal pain.
Gastroenterologist after CT scan says since bowel is infected, it has to be operated immediately and chances of survival is very less-10% , even if he operate there is no guarantee.
We are confused now, and need further opinion as to :
- Is it possible to apply both the procedure in a single sitting ie putting graft and bowel corrections in single sitting ?
- Will SMA works fine after Bowel corrections or SMA block to be removed only by grafting Aortic dissection ?
Kindly advise.

If the bowel has got infected then it has to be operated urgently and no other procedure can be done as there is source of infection inside.
Aortic grafting cannot be done as of now and the SMA will be taken care of during surgery most likely.

Just last clarification..
is obstruction/blood clot in the SMA gets resolved by surgical removal of the bad part of the bowel ? after surgery SMA starts supplying blood to small intestine without further causing damage to the bowel ?

When the surgeon is opening the abdomen for infected bowel removal he probably will also treat any blockage which he knows is there in the SMA.
Rest I would like to suggest you to discuss this with your surgeon as what will be his plan of action on this as he will tell and guide you better in this regards.
