What Causes Abdominal Pain While On Invokana?

Question: Walking between 5,000 and 10,000 steps a day, diet is well under control. Blood sugar readings in April ranged from 13.5 up to 19.5, fasting. Priority was given to controlling hypertension, and potential gastric bleed.
Family doctor started me on Invokana 100 mg., last week. There has been NO change in the readings, this morning was 13.9
Upper abdominal pain continues, but it does NOT feel like the previous bouts with pancreatitis.

I carefully passed through your question and would explain that abdominal pain could be related to Invokana adverse effects.

Did this abdominal pain start when starting Invokana. In such case it could be an adverse effect related to this drug. If you do not tolerate this abdominal pain, you may need to stop taking it and switch to another antidiabetic drug.

From the other side, as your fasting blood sugar values have changed, you may need to increase the dose the dose of this drug or switch to another drug (if you do not tolerate it).

Anyway, I would recommend consulting with your doctor and perfoming an abdominal ultrasound and some blood lab tests (complete blood count, amylaze plasma levels, kidney and liver function tests), in order to exclude other possible caused of abdominal pain.

If other possible causes a excluded you should consider abdominal pain as an adverse effect of Invokana and discuss with your doctor on the above options.

Thank you AGAIN, Dr. Sharka. If I make it through all this, you will have made an excellent contribution.
Blood work was done yesterday, this time they are actually checking enzymes and iron. Had a previous iron sat of 0.09, understand that is low. Abdominal ultrasound has been ordered, they are also checking kidneys.
I do see my new EXCELLENT family doctor on Friday am. The symptoms have worsened since starting the Invokana. If the pain increases, I will go to emerg. Eating produces painful distension and bloating, but fluids will do until Friday. Never been this thirsty in my life, cannot be quenched.

Thank you for the additional information.

The fact that your feel thirsty during all the time may indicate possible electrolytes imbalances or a hormonal imbalance. Uncontrolled diabetes and hiperosmolary conditions could lead to increased thirst.

What about urination? Do you urinate a lot after drinking a lot? Have you gained any pounds during these days? Increased thirst and weight gain could be related to heart failure. I would recommend performing further tests like a cardiac ultrasound, NT Pro BNP levels and some other blood lab tests (blood electrolytes, blood osmolarity) besides the above mentioned tests.

The increased abdominal pain and abdominal extension after eating could be explained by gastro-duodenal active ulcers, which get worse when the stomach needs to digest the food by increasing acid production. A new fibrogastroscopy may be needed.

The low iron levels coupled with anemia, could indicate need for iron supplements, but they are not related to the abdominal pain.

You should discuss with your doctor on the above issues.

Again thank you. I was diagnosed in acute heart failure in 2012, post pneumonia, successfully resolved.
Looking down, my ankles are swollen in spite of two drugs for hypertension. I looked up "hiperosmolary conditions", and am now concerned. There has been muscle cramps that past two weeks, but has been exacerbated since beginning the Invokana. The cramps are relieved by the 50 mg potassium tabs I have on hand.
I think it is time to go to emerg...actually, I know it is time to go to emerg. The clinic wanted me to do that yesterday, but I wanted to touch base with my family doctor on Friday.
The time you took to write and explain things has made the difference.
I'll print this out to take with me.

I agree with your decision on going to the ER. It is necessary a careful physical check up and the above mentioned tests. It is true that electrolytes imbalances can cause muscular cramps. Anyway all the above suspicions should be ruled in/out.
