Question: hi, last september, i had a week of intense abdominal pain every morning until after i ate. i was doubled over, in a lot of pain. the doctors told me it was gastroenteritus and i should wait. it got milder, but i am still unable to sleep on my stomach every night. i have to lie face up, or if sideways i need a pillow for support. i am unable to put pressure on my abdomen. the pain is mainly upper abdomen and central, above the belly button. but can feel diffused. this is constant. in addition to this, every month or so, it flares up and becomes more extreme. returning to the original cramps, or with twinges of pain elsewhere. the last few weeks it has got a lot worse, with a period where i had chest pain, then extreme acid burn in my throat, and now also body odour problems which i've never had before. i have also had pain after ejaculating since october though not sure that is connected. the doctors say my blood test is clear.

Brief Answer: Duodenal ulcer is the first possibility Detailed Answer: Hi. Noted your Medical history of mesenteric adenitis in childhood. The pain is more in the morning, severe, in the upper abdomen, flares up every month, accompanied recently with a chest pain, acid burn in throat and body odor. I would suggest in such a case the following investigation for the diagnosis and/ or to rule out the following way. Upper GI Endoscopy to start with to see if you have an ulcer in the stomach or duodenum and signs of healed/ old ulcer. It also shows the condition of the esophagus. Ultrasonography of the abdomen after a good preparation like giving charcoal tablets and dulcolax to clear the bowel a night before and to keep you nil orally after 10 pm the night before. This will help to diagnose problems of pancreas, liver and gall bladder along with other organs and associated findings of lymph nodes and fluid or mass in abdomen. If the sonography is suggestive of some positive findings , I would advise a CT scan to confirm the diagnosis. It is good that your blood tests are clear; I would also go for a test of stool- routine and microscopy and for occult blood. Pain after ejaculation may not be directly related to the present problem. I hope this answer helps you to get a proper diagnosis , thereby helping to get the proper treatment for cure.

Thats great, thanks for the response. This seems to correlate to my research on my symptoms. My doctor here is reluctant to give me any antibiotics or do a stool test, saying that the first course of action should be the colonoscopy. Do you agree with this? Surely a stool test could quickly check for Helicobacter Pylori or other possible issues? I am convinced that this was due to something I ate - as initially, when it started, it was after I had been berry picking. I had dirty hands as I was also picking flowers, had touched roots and mushrooms... Also, some of the berries tasted bad, and of course, were unwashed. I had this sort of dismissed by the doctor here as a possible cause of illness. Do you think that this could be a possible cause? Also - the pain has been mainly after I ejaculate but is now also after I urinate sometimes. It happened after using an overly tight condom, and after refraining from sexual activity for a short period. I initially thought it was just some kind of strain, but then it has remained since last October.

Brief Answer: Go for colonoscopy to start with/ Urologist' cons Detailed Answer: Fine. Go for a colonoscopy, the transverse part of the colon lies in the same region you described to have pain. And your Doctor must have done a clinical / physical examination. If colonoscopy is normal , he would definitely advise you Upper GI Endoscopy. Your Doctor may be reluctant to give you antibiotics as taking them will not give helicobactor test positive. It will be difficult for me to comment what your Doctor would actually be thinking. Other than the explanations you have given as a probable cause there are many other causes , so let us not go into details as the diagnosis and proper treatment is more important at the moment. The pain on ejaculation and now after urination look to be due to prostatitis, for which you need to be checked by digital ( manual) examination , preferably by an Urologist and Ultrasonography will also help for the proper diagnosis. Visit an Urologist as you have problems since since October.
