Question: Hi ,

My wife aged 36 has speaking problem for the past 2 days(in the left side), speech is slightly jumbled, she feels here tongue & swallowing sensation in the left side throat is numb,

She doesn't have any other difficulties in here left side of the body,

Having speech problem and abnormal swallowing and numb sensation in throat can be due to a stroke like episode. It is also possible that there is some problem with speech center or nerves connecting the tongue to the brain and this needs to be investigated in detail. She requires to be seen by a neurologist immediately and a brain CT or MRI scan as required after clinical examination.

Once the conditions in brain are evaluated then other conditions like electrolyte imbalances or metabolic causes need consideration.

Though age is 36 years, any stroke like event or abnormality in brain cannot be entirely ruled out without clinical examination and imaging investigation.

We did MRI and the report suggests that this could be a multiple sclerosis condition,

How serious is this and what are the further investigation / treatment to be done?

First it is important to discuss the MRI scan with the neurologist and confirm the diagnosis.

This is a condition when there is damage to the covering of nerves in the brain and spinal cord. The communication between nerves is disrupted and the patient has symptoms depending on the location of nerves affected.

It is important to observe the symptoms closely and take treatment as given. To confirm multiple sclerosis a repeat MRI scan is important with comparison of findings. There are diagnostic criteria on MRI scan which are to be followed before confirming the diagnosis as multiple sclerosis.

If possible, I would like to see a copy of the report to know extent of the problem.

Many patients respond to treatment with steroids and disease controlling agents. The symptoms of multiple sclerosis can change over time and the patient develops new symptoms. The symptoms are nerve related and improve or worsen with time. There are periods of improvement followed by increase of symptoms.

Remaining stress free and doing regular mild exercise helps in maintaining the general health of the patient.
