What Causes Acne On Forehead And Nose?

Question: Dear Dermatologist
* basically my skin is oily. on forehad, nose oil formation will be there but these days i am observing that my cheeks, chin is getting dried & rough. also on forehead lot of small black dot marks are there. may be old pimples left that marks. along with that i am getting new pimples sometimes. sometimes pimples dry in a day & disappear, sometimes stays for a week & gets bulged. it leaves mark later.

* I have observed small pigmentation kind of color change around my nose. doctor said something as butterfly kind of mark or some medical term. he also told its common & mine is not that worse. is it so?

* my skin wont look clear & smooth. is there a permanent solution for this?

* I consulted a dermatologist couple of days back & he suggested some creams. I HAVE ATTACHED HIS PRSCRIPTION. but when i used it for two days i felt my face is getting red & looks like marks are more visible. What will u advice on ths? these ointments are costly but will go per your guidance

pls answer all the above pointwise & answer all my questions please. PLEASE ANSWER ALL MY QUESTIONS IN DETAIL & POINT WISE BECAUSE I AM WORRIED. IF I HAVE TO CONTINUE THE SAME CREAMS AS MENTIONED IN PRESCRIPTION PLS EXPLAIN IN DETAIL HOW TO APPLY WHEN TO APPLY ETC. IN DETAIL.

Hello. Thank you for writing to us

I have gone through your query and I have viewed the attachments.
I could not clearly make out the pigmentation around nose area in these images. You may upload more images of that area.
You have inflammatory acne i.e red and pustular.
Ideally I would suggest an oral antiacne antibiotic e.g either doxycycline, minocycline or azithromycin in addition to the topical antiacne medication. You may start with Cap. Microdox LBX 100mg (Doxycycline), twice daily for 2-4 weeks. It may need to be continued further.
Seems topical benzoyl peroxide gel (persol 2.5% gel) that you have been using is too irritating for your sensitive skin and therefore you may discontinue it. Discontinue Aziderm 20% gel as well. Instead you may use Nadifloxacin 1% gel (Nadoxin gel), twice daily for your active acne spots and Deriva-CMS gel (Adapalene 0.1% and clindamycin 1%) for night application.
Use a gentle cleanser for face wash e.g Cetaphil OS cleanser. It is mild as well as removes excess oil therefore will suit your sensitive yet oily skin

ok i will try these. please explain how to use


(1) "Microdox LBX 100 mg" when to take & is it after food or before food
(2) (a) You told to apply "Nadifloxacin 1% gel (Nadoxin Gel)" twice a day & (b) Deriva-CMS Gel (Adapalene 0.1% and Clindamycin 1%) for over night application. but if (a) to be applied twice a day, how & when to apply because morning I will be going to a coaching class & then to work. THANK YOU

Hi.

Microdox Cap. is to be taken 2x daily, morning after breakfast and evening after dinner.
You may apply Nadoxin gel only once daily, every morning before going out. On weekends and on off days you can apply it in afternoon as well
Deriva CMS gel is to be applied once daily at night.

in line to above mentioned problem-

I have been using Microdox LBX 100 mg, Nadifloxacin 1% gel (Nadoxin Gel), Deriva-CMS Gel (Adapalene 0.1% and Clindamycin 1%) regularly since the day u advised. after using for 4-5 days i was thinking like marks are reducing. but i feel my skin has become dark & small small dots on face. pleas see the photos

Now I have observed that my skin has lost its smoothness compltely i think. pls see the attache images. face is looking dark & kind of small smal dots on the whole face (small like rawa /sooji size). on forehead, cheeks, under the lower lip. also pores on cheeks looking so bad sir. look at the images.

is it normal ? will my face become normal? will i get back my complexion?

Regarding acne remedy

Detailed Answer:
Hi.

These small dots are acne and there is some irritation. Don't apply too much of topical medications. It can cause irritation. You have to just apply a thin film and that's it.
It is normal to expect some dryness/ peeling/ roughness with these medications particularly Deriva CMS gel. That's how it peels off acne.
I suggest you to stop using Deriva CMS gel for 4-5 days. Just use Nadoxin gel, twice daily and once your skin looks better you may restart Deriva CMS gel for every alternate night application.
Use a gentle cleanser for face wash i.e Cetaphil gentle cleansing lotion, twice daily.
