I am Dr XXXXXXX Ahmad. I read your question and will try to help you in this regard.
Pain in bowel movement with fresh blood is a typical indication of anal fissure.
It got cured that is indeed great but according to your history and description there is still some irritable focus at the point where fissure was.
Feces are full of germs so no wonder an exposed cut can get inflammed.
I would suggest you some measures which will benefit you quite much.
It will not take more than a fortnight of religiously controlling your life style, to control the problem.
Main thing is to control your constipation...
your stools need to be soft otherwise this will not settle.
For this you can:
a) increase fibre intake in diet..switch to whole wheat products..use fibrous vegetables,
Drink good amount of fluids..reduce meat content in your foot for the time being..avoud junk.
2) You can use OTC lexatives of medoun strength to keep stools soft.
3) try not to strain much while defectaing.
4) In my clinic i would suggest Lignocain or tronolene haemorhoidal gel 6 hourly it saves damaged skin from germs and also relieve pain by numbing the area so reduce the spasm of the sphincter.
5) I would also suggest my patients to use amoxicillinn500mg twice a day fir five days as an antibiotic cover ... but as you are not bleeding now so even without it there should not be any problem

One more issue is there that I feel some small muscular outgrowth near my anal portion , probably 2 small outgrowth, and little bit of swelling near my anal portion , but it is not painful.Only during constipation, it swells and become painful.Is surgery final remedy for it, or even after surgery it may reoccur

Hi,
Permanent swelling may mean healed up anal fissure, healed fissure can also pe represented by a skin tag,It can also be growth of sphincter muscle ,,all these are complications of constipation
And the soft swelling (which mostly feel like water filled baloon ) are usually 1st degree haemorrhoids which may swell up during hard stool passage and straining but they go back and swelling disappears after 5-10 minutes of stool passage.
For 1st degree haemorrhoids you don't need any surgery one just has to eat more fibre,
Drink more water,
avoid things which cause constipation , may be use a mild laxative,
use vegetables,
If you need detailed account of food and diatry modifications i can give you that too,
Even for muscle growth nothing has to be done if it is small...What did the surgeon advise? did he advise surgery?
