Dear Mr XXXXXXX
Let's analyse your complaint
1. Irritation
2. Muscle stiffness
These complaint suggest the possibility of anal fissure. Following are the points suggestive of diagnosis
1. Pain/ Discomfort: anal fissure is a small ulcer at anal opening. So it causes pain and discomfort while passing stools as it stretches th ulcer
2. Spasm: Fissure causes reflux spasm of internal anal sphincter
3. Hemorrhoids are usually painless and just cause painless bleeding in stools
Presence of pain while defecation and associated with bleeding one should consider the possibility of anal fissure
Following are the predisposing factors for occurrence of anal fissure

2. Reduced intake of water
