What Causes Anal Fissures?

Question: Hi, I am 44 and have had anal fissures for almost 20 years. Slight blood on paper when using the bathroom has now progressed to blood in the toilet at times. This does not happen all the time it seems to occur more often when force is used. This morning passed gas while urinating and had some blood that I did not notice that stained my shorts. Blood is bright red did notice a small clot when I wiped myself after noticing some blood had passes through my shorts. I had my wife do a visual and she saw two distinct fissures half of a centimeter in length. In the past I have had times where B.M.s have caused severe burning on anal area that's lasted for hours. This issue seems to be progressing but blood does not pass on every B.M. and stool looks normal usually. Have eaten spicy food lately and a lot of coffee daily other than that eat pretty healthy decent amount of fiber usually. I have been sitting for extended amounts of time lately...any advice? Thank you very much...

Hi Sir,

Welcome to HealthcareMagic .

Fissure , the main cause is constipation. Even slight strain in passing stools stretch the anus and bleeds and pains. So first and main thing is to avoid constipation. This may be done by taking laxatives and high fiber diet with green leafy vegetables.

Taking too spicy foods increases blood flow to anus and more bleeding. You need to have regular motions daily. Standing of stool for longer time in intestine make it more harder by absorbing water.

For the present fissure to heal you need to apply combination ointment like zinc oxide, lignocaine, steroid and calcium dobusolate . These all have different action. Reduces burning pain, odema and helps in healing. Along with this I suggest to do sitz bath . In which you need to sit in a tub with XXXXXXX warm water with 2tsp dettol for 20 mins 3 to 4 times a day. You may discuss this with your doctor. This will reduce the tone of sphincter and reduces bleeding.

If you are already doing this from many days with no improvement then you need to discuss regarding need for surgery with your surgeon.
