I have reviewed this information as well as the attached image and it is suggestive of a wound at the anal orifice. The secretions from this certainly cause huge irritation and explains the excessive itching. I will recommend Sitz baths: In a bowl containing 1.5liters of lukewarm water, add 15ml of Betadine or any other antiseptic to it. Sit in it for 15minutes, at least thrice daily and after every stool. This will help much and speed up recovery. Itches will also decrease significantly.

I looked at the wounds and that wasn't suggestive of herpes 1. Herpes would have given multiple ulcers and that wasn't what we found on the image. I understand your worries but will want to wait a little longer to gather more details before concluding on such.

Flare ups vary if this is herpes. We simply need to really know what the problem is and we will be able to address better.
