default
Posted on Wed, 14 Jun 2017
Question: was diagnosed with UC in november i am on colazal 9 a day, bentyl before meals and at bedtime and colestipol two a day. Still have diarhea every morning and after i eat. sometimes urgent! the last two days i have had severe lower back pain is that because of the uc? I did have gall bladder removed and also had pancreatitis about 15 years ago. Any suggestions? i am also on the last week of prednisone treatment. That seemed to stop the bleeding but not the diarhea. I am 65 year old female.
my lower stomach and back hurt all the time my lower stomach is very tender. when i sit i feel like i'm sitting on steel rod shooting up me. somedays i can eat a yogurt and be ok other days eat a yogurt and am in the bathroom with in 10 minutes Why is that?
that is what it/s like with all foods and time of day. Nothing is consistent. My doctor is unavailable until july so likely not going to be able to switch meds. i have also tried over the counter probiotics but didnt' make a differencen I also can't seem to lose weght i weigh 142 and have never weighed this much. my stomach seem to grow as the day goes on and very un comfortable

Your answers are as follows.

Detailed Answer:
Hello,
Thanks for posting the query on this forum.
I've gone through your details and I appreciate your concerns.

Answers--

1) Back pain in a patient with ulcerative colitis can be a result of ankylosing spondylitis.Patient with ulcerative colitis are candidates prone to ankylosing spondylitis. However, initially, a lumbosacral spine X-ray can be done to rule out any spine problem.Sudden weight gain can also be responsible for back pain.

Loose or watery stools occur when the intestines push the stools through the bowel to be eliminated before excess water can be reabsorbed. As in UC, intestines are inflamed hence absorption of water is improper causing stools to be loose. For relief in loose stools start taking Imodium 4mg twice daily(OTC).

Though a number of other drugs are available but Imodium has been safely used for years to control diarrhea in patients with ulcerative colitis.

Natural remedies-
Peel and slice or chop 1-2 apples. Place in a small saucepan with about ½” of water, 1 teaspoon olive oil, and a pinch each of Cinnamon, Cardamom, Nutmeg, and Natural Mineral Salt. Bring to a boil and then turn down to a simmer (if you need to add a bit of water, please do). Cook covered until the apples are mushy (at least 30 minutes). The apples are gently astringent and help to bind the stools.

Chop 1-2 ripe bananas into bite-sized pieces. Garnish with 1 teaspoon warm olive oil, and a pinch each of Cinnamon, Cardamom, and Nutmeg. Have it a cup a day.

Answer 2) One of the problems that can occur with UC is a dietary allergy to some foods.
Almost 50% patients of ulcerative colitis are allergic to dairy products.

In a recent study conducted by a group of 75 patients, 30.6% have mild, 52.8% moderate and 16.6% were in severe allergy to certain foods especially dairies. You have to find out the food which triggers up your problem and has to avoid them completely. Avoiding certain foods (e.g., dairy products, cabbage, broccoli, beans, spicy foods) and increasing dietary fiber, may help in your case. However, at the end of the day, it's only you who have to find out to which food is your body allergic.

As you are experiencing severe tenderness in the abdomen, an MRI abdomen should be done to rule out any new problem in intestines. However anti-inflammatory like Tylenol can be taken on as and when required basis (not more than three-four times a day) to alleviate symptoms of pain.

Start moderate exercise daily for at least 30 minutes to loose weight.

my gastro dr is unavailable for 12 weeks and I am not sure if my family doctor would order these tests and not sure if insurance will cover. but I will try to find out.
would taking any vitamins help?
do you think the collazal, bentyl and colestipol should be working? This is very hard to handle because I do not go anywhere for fear of having an episode that I canT control. I will try immodium and see if that helps.
I am really not able to exercise to much because of the copd I get out of breath real easy.
Thanks for the answers so far, I appreciate your time.

Brief Answer:
Add loperamide to your regimen.

Detailed Answer:
Balsalazide is an anti-inflammatory drug used in the treatment of inflammatory bowel disease.
Bentyl-Dicyclomine is an antispasmodic drug and usually used to relief abdominal pain.It helps to reduce the symptoms of stomach and intestinal cramping.
Colestipol is a lipid lowerin drug with side effect of constipation. We are using this side effect to help you.

However loperamide (over the counter)-Loperamide is an opioid-receptor agonist .Loperamide works similarly to morphine, decreasing the activity of the myenteric plexus(intestinal nerves), which decreases the tone of the longitudinal and circular smooth muscles of the intestinal wall. This increases the time food stays in the intestine, allowing more water to be absorbed from the fecal matter. Loperamide also decreases colonic mass movements and suppresses the gastrocolic reflex. It is a strong constipating agent.I would strongly recommend you to start it initially at 4 mg thrice daily doses.

Vitamins will be good for overall health but i don't think they can help in primary symptom.

Initially start taking imodium and tell me how you feel after a week.
Please mention the dose and frequency of all three drugs you are on.
waiting.

Brief Answer:
Follow up.

Detailed Answer:
Hi again Dear,

Everyone's body is a unique system in itself. What a doctor studies, is the common features of a problem termed medically as 95 percentile(which means that in 95 out of 100 patients suffering from particular problem certain symptoms were found hence they are considered as chief symptoms of that problem despite the fact that rest 5% does not have the same symptoms).

In the same way why your symptoms get worse in morning is something which is unique for your body and a valid reason can be given only after doing a battery of investigations on you (medical school case).

There is no textbook explanation for this question. However, at the end of the day, treatment remains the same.

So,
I'd suggest continuing the Imodium 4mg thrice daily(kindly follow the dose suggested) and follow up after 5 days.

Warm regards.

Detailed Answer:
Hi again,
Taking probiotics are always good as they improve overall digestion.Infact you are on a number of medicines so you should take a probiotic daily.

No we would taper off the medicines as per your response to particular medications. However some medicines may be required life long also.

Initially let me first see how you are responding to imodium.

Regards.
