Brief Answer:
Probably you are having painless bleeding in the urine

Detailed Answer:
Hi!
Welcome to HCM!

I can appreciate your concern for observing blood stained underware.
In view of absence of other symptoms like pain during process of urination, burning sensation, flank pains, fever or taking any medicines like blood thinners I am of the opinion that the bleeding might be originating from some problem either from your kidney, ureter (the pipe which drains the urine from kidney) or urinary bladder.

My advice is get the urine examination and the ultrasound done to find out the possible causes like stones, infections, or tumors from the urinary tract.

Looking forward to listen more from your side.
Take care, drink a lot of water.
