What Causes Blood Clots In The Lungs After Hip Replacement Surgery?

Question: 81 y/o patient with a recent 3/1 hip replacement - post operative ileus did not resolve at extended care facility. Moved back to the hospital 3/18/17 discovered blood clots in both lungs and in lower abdomen and left non surgical leg - elevated wbc upon arrival, lactic acid normal but air pockets in large intestine on CT scan - iv fluids and antibiotics started and heparin after surgery was declined.

today is 3/22/17 - planning on colonoscopy tomorrow morning

After carefully reviewing your clinical history, I would explain that a colonoscopy is not the best exam to do during this period of time.

You should know that colonoscopy can be a little traumatic for the bowel, which imposes you to a higher physical stress.

Besides the fact that you are taking a blood thinner, imposes you to a higher risk of bleeding.

Coming to this point, I would recommend performing other alternative tests such as irrigography (radiological exam of the colon with rectal contrast) or an abdominal CT scan with oral contrast, which help examine the inner layer of your colon and help find this possible cause of ileus.

You should discuss with your doctor on the above issues.
