Blood in semen can be benign and not indicative of any medical conditions.
Sometimes rigorous sexual activity, trauma, medications like aspirin can lead to blood in semen
However, since you are 57, I would recommend further evaluation to look for any infection, tumor, prostate enlargement or hypertension as the cause of your symptoms.
Since your symptoms are associated with testicular pain, infection of the epididymis (Epididymitis) is a possibility.
Physical examination, rectal examination, urine analysis, culture and imaging studies are needed to make a definite diagnosis.
