Blood in the semen is called hematospermia.
In the majority of men, there is little clinical significance , meaning that it is not associated with any significant medical condition, it clears on its own within 2 months.
In some cases, there may be an associated condition that needs to treated.
Some of the causes include--- (1)infection of the prostate which may require antibiotics
(2)benign (not cancerous) enlargement of the prostate
(3)malignant enlargement of the prostate
(4)other infection of the urinary tract
(5)cysts
(6)severe high blood pressure, liver disease or a bleeding disorder
A urine analysis and culture (looking for bacteria)-- would determine if there is an infection.
Imaging, such as ultrasound, would determine if an ultrasound is needed. This would be done , for example if the examination suggest an enlarged prostate.
