Dear XXXXXXX
Greetings from health care magic!!! I am Dr Thakker and shall be answering your question.
I can understand your anxiety about the blood in urine.
After reading the clinical history, the following are the possible causes of blood in the urine:
Traumatic catheterisation: The urethral epithelium is very delicate and can be damaged if the catheter is not inserted correctly. If a catheter of a large size is used, it can cause bleeding as well.
Urethral stricture: Chronic urethral catherisation leads to narrowing of the urethra. This can bleed during insertion.
Urethral stone: This can cause severe pain and blood in the urine.
Bladder malignancy: This is a remote cause in him given he has bleeding after catheterisation.
The next step is to get a retrograde urethrogram and diagnose the cause of bleeding. Platelet counts, APTT , PT/INR should also be done. I would recommend you to rush him to the ER because we don't want him to go into shock from blood loss.
I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.
Yours sincerely,
Dr Thakker.
