In my opinion you should consult pulmonologist and get done clinical examination.
Young female with PAH needs following things to rule out.
1. Connective Tissue Diseases
2. HIV
3. Storage diseases
4. Family History of PAH
5. Valvular Heart Diseases etc
And if found anything than treatment should be started accordingly.
Also get done CT Pulmonary Angiography to rule out Pulmonary Embolism as a complication. Worsening of symptoms may be due to this only. So get done CT Pulmonary Angio.
Very few drugs are available for this disease. And you can add sildenafil,along with bosentan and pulmopress.
Diuretics and oral inotropes can also be tried.
So better to consult pulmonologist and discuss all these.
Honestly speaking PAH is very bad disease when it comes to prognosis. Very few treatment options are available. So In my opinion these symptoms are not good sign.

PAH is chronic disease. You need to take these drugs lifelong.
It is not curable; it should be managed through life time medicines. So treatment lasts long.
And patient needs psychological support too. With lifestyle modifications (chest physiotherapy, low salt diet) and proper treatment life expectancy with PAH is good.
It basically hampers quality of life. Five year survival with best treatment is 70-80%. But patient becomes physically limited.
And psychological support is must.
Life expectancy differs patient to patient.
So my advice in your case is, first stabilize her and then start regular maintenance therapy.

Hello dear, Thanks for your question on HCM.
If any patient is under going treatment for any disease, the main expectation of that patient is to be free from the disease if possible or to live his /her life independently with full activities.
This can be true for any chronic disease like diabetes, hypertension etc but not with the PAH.
Patient becomes so dependent on oxygen that he or she has to carry oxygen mask almost 17-18 hours a day. He can not walk or jog as efficient as in the past. Breathlessness restrict all his activities.
In end stages patient feel breathless even on eating and drinking.
So the word Quality is used for all these routine activities. PAH patient can not do his/her routine day to day activities with out oxygen support.
Death is not sudden in PAH patients. They become bed ridden in early stage of disease and remains bed ridden there after. So quality of life is miserable but 5 year survival is good with treatment. But in the bed only.
