Brief Answer:
saline lavage/rinse and antibiotics may be helpful

Detailed Answer:
HI, thanks for using healthcare magic

It is possible that there may be a bacterial or fungal infection present, causing the odor.
Bacterial infections are more common than fungal infections , fungal would be considered if there is no response to treatment of the bacterial infection.

He may need a course of antibiotics and may also want to try saline lavage/rinse as well.
Saline lavage/rinse would help to clean out the sinuses.

Saline rinse devices can be found at many pharmacies over the counter.

The antibiotics would require a prescription from his doctor.

If this is not an option at the moment then you can try the rinse only and monitor

I hope this helps, feel free to ask any other questions
