Brief Answer:
Oral antibiotics, antifungals, topical triple combination cream
Detailed Answer:
Hello,
Thank you for posting the pictures.
I have reviewed them keenly.
It seems you have got scrotal dermatitis with secondary Intertrigo.
Excess sweating coupled with friction in groin area can trigger inflammation in tender skin of scrotum leading to dermatitis called scrotal dermatitis and which in turn can get infected with bacteria or fungus leading to a condition called Intertrigo.
In such cases, I generally suggest you oral antibiotics like erythromycin for 7 days and antifungal tablets like fluconazole for 7-10 days along with application of triple combination cream containing corticosteroid like betamethasone, antibiotic like fusidic acid and antifungal like clotrimazole twice a day for 2-3 weeks.
Use dusting powder containing clotrimazole on daily basis to keep area dry and reduce friction.
Try to reduce weight and maintain hygiene.
Avoid tight undergarments and use cotton garments as far as possible.
Hope this will help you
Take care
