Brief Answer:
Possibility of rotator cuff tear

Detailed Answer:
Hello,

Thanks for writing to us,

There is possibility of rotator cuff tear leading to bulge and swelling.
You may need Ultrasonography or if possible MRI.
Another possibility of infection is likely diagnosis
DO NOT IGNORE INFECTION IN POST joint replacement.
You may need to investigate like CRP, CBC, ESR
PCR culture is very useful to find culprit organism.

If infection is confirmed then debridement is required.

Rotator cuff injury may require physiotherapy and exercises if minor
If major tear then arthroscopic repair may require.
Some time metallosis leads to foreign body inflammation that can lead to swelling.
