Question: I am facing problem of burning in anal area after passing stool. I had visited doctor he said it looks like starting of piles and had prescribed Anovat cream and advised to changes eating habits. My weight is 95 kgs and height is 5ft 7 inches. I started eating fruits fruits and green vegetables and was feeling much better. But the problem is even I eat very little spicy next day I find excessive burning and itching after passing stool. I had one drink yesterday and very little starter today I faced tremendous problem of burning. Will there is any permanent solution for this. Will I have to go for surgery ? Symptoms of blood is never observed in stool, I use western type toilet as there is no provision of XXXXXXX toilet. I am applying Anovat since last one and half month

I have thoroughly read your question and completely understand your concern.
Your history(absence of bleeding etc) is suggestive of grade I piles.Good news is that grade I piles is completely reversible and does not need surgery.

I would be glad to know some more information.
Do you strain during stools?Are you able to completely empty your bowels?
Is your stool hard?
Are your bowels regular?
Which toilet have you been previously used to?Is there any change recently which has affected your bowel movement?

Sir I do strain during stools. No my bowels are not completely empty. I pass stool thrice in a morning. First when I wake up with 5 to 15 mins I pass the stool which is with pressure but is not complete. After that I take tea and within 45 to 60 min I again pass the stool. Two times in the morning is sure sometimes it is three times also. In afternoon after taking lunch again I pass on the stool within 30 to 45 mins. Bowel movements are regular but thrice a day. Stool is hard. But since last one and half month facing problem of excessive burning and itching after passing of stool . I use to go for walk in morning after passing stool for the first time but during walking face terrific itching in anus many times and feel like rubbing it with hand. I started taking rich fibre food few days it was ok but not completely as I was still facing problem of itching. I am applying Anovat cream since last one month. I am using western toilet since last 20 years. I do not understand it is piles or what or is it IBD ?

Hi dear.Thanks for writing me back and sharing the information.

The fact that your stools are hard is suggestive of less fibre and reduced hydration.I am glad that you have made an attempt to add fibre to your diet and started exercising.My sincere advise is that it should be a regular habit to incorporate these measures in your lifestyle & they shall definitely help you overcome your difficulty.

Do's:

a. Add more legumes, whole grains, seeds, vegetables and fruits.You can also try psyllium, ispaghula husk fiber, methylcellulose(supplements like Citrucel) available over the counter.These add bulk to your stool and help emptying.

b. Stay hydrated.Drinking water(or any other fluids) shall soften stools, making them easier to pass and reducing the urge to strain, which can potentially worsen your hemorrhoids. Increase intake to eight to 10 glasses of water a day.

c. Keep the anal area clean and dry.Wash it gently with a soft washcloth, warm water and mild soap.

d.Get more exercise.Exercise stimulates bowel function, which makes it easier to pass your stools.At-least 20 minutes a day of moderate aerobic exercise is recommended.Continue walking if you have already started.Drink about 2-3 glasses of water as soon as you wake up.Hit the gym or walk regularly after you wake up, post which you will notice that this has eased your bowel movement.

Dont's:
e. Foods to omit or reduce: fried foods, refined carbohydrates, dairy products, sodium.

Thus exercise, high-fiber diet and drinking lots of water not only helps in stool movement but also prevents hemorrhoids from returning.

I were to examine locally (around the anus) to rule out yeast infection(commonest cause of itching).This is the common cause of itching. Anovate cream do not cure yeast infection.Avoid using dettol.Toilet is not a concern since you have been used to.It is definitely not IBD.
