Question: I had a sexual encounter 2 months ago with someone who want my spouse. Had a burning feeling on penis Went to doctors got treat with doxycycline 2 weeks flatly 1 dose. Urine test showed few leukocytyes. Continued having the issue so went to another doctor I got treated with ceftriaxone and 2gm azithromiacin. Then few days later testicles became enlarged and painful. Went to doctors and got put on levaquin for epididymitis. I got treated during and After being on antibiotics. And all came back negative... After the levaquin things cleared up. 3 weeks later had sexual relations with my wife and the pain and burning along with swelling once againleukocytes.

Could I still have epididymitis even though test show no stds like gon or chlamydia. Or am I in the clear. Still having same symptoms as before. Testicular.pain burning tip and general I'll feeling with nausea. By the way I went to a urologist and he checked my prostate and said it was fine.
Today I seen another doctor that told me I had epididymitis..but I have no sass. I'm confused and scared that I have given my wife somethimg

Brief Answer:
Need to evaluate properly .

Detailed Answer:
hello
Thanks for query .
Based on the sequence of events that you have narrated I would state that you have developed infection of Genito Urinary system after having unprotected sex with a woman being unaware about her STD status .

Normally it takes 2 weeks for STD to be detected positive after exposure .However since you were taking antibiotics and that may be the reason for these tests to be negative .

I shall be happy if you can post me more details to help me in making precise comments
1) Have you done Ultrasound Scanning of Scrotum ?
2) Have you done urine culture and were being prescribed antibiotic as per culture report?

Normally it takes 4-6 weeks for Epididymitis to get resolved hence one has to take appropriate antibiotic as per culture report for 6-8 weeks along with anti inflammatory drug like Diclofenac twice daily.

I would suggest you to repeat following basic tests and take appropriate antibiotic as per culture report .
1) Urine culture
2) Ultrasound Scanning and Color Doppler study of Scrotum.

Ensure to have safe and protected sex to avoid infection being transmitted to your wife .

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Dr. Thanks for the reply. I had the ultra sound and it was normal. Then two days after started having pains once again. But I've had a urine culture and sensitivity tests and they've both come.back negative for no bacteria. They've also checked regular blood and urine for stds and all negative. This was just recently 3 weeks after last antibiotic. So dr/surgeon tells me I'm ok yet another doctor says I have epididymitis still although urine culture shows no infection and no bacteria have been found. This is all confusing. Could I possibly have a bacteria that's not detectable like m. Genetaliun, could I possible have a chemical epididymitis. Or could it be epididymitis from an std that wasn't picked up although tests are neg. Is there another exam I should be requesting. Everything they check is neg. Should once requesting other exams tonchecknfor hernia, etc. Maybe it can all be coincidental this happened around the time I made a mistake.

Brief Answer:
Likely to be non bacterial Epididymitis

Detailed Answer:
hello
Thanks for follow up and highlighting details of the test you have gone through .

In absence of any positive evidence the pain in scrotum could be due to inflammation of Epididymis (Epididymitis).

Take treatment suggested along with Diethylcarbamazine (Hetrazan ) twice daily .

In absence of you to have proved STD it is unlikely that your wife will get it transmitted from you through sexual intercourse .

It is your anxiety that is bothering you.Stay calm .

Thanks and Regards.

Dr. Good afternoon. Few more questions.
As of right now I may have a active infection of epididymitis. Can I be intimate with my spouse because all the tests are negative?
Also can I still have active bacteria in my epididymis that is std related and it just wasn't picked up by the urine culture?
Or because the tests are negative does that mean I'm in the clear?

Lastly should there be an additional test I should ask for to check for bacteria that may have been missed. Like checking my sperm?
