What Causes Burning Sensation At The End Of The Penis Along With Urethral Bleeding?

Question: I have urethral bleeding that burns at the end of my penis. Can you tell me the possible causes?
History: I am 76. I had my first urethral surgery for a stricture (possibly from a bicycle straddle injury as a teenager) in 1965. For the last 50 years, as scar tissue kept building up and restricting urethral flow, I have had periodic dilations and at least 6 more surgeries, including cold knife, laser and skin graft, none of which were successful long term. I did my own dilations weekly for the last 25 years. I have had no urethral problems since Dr XXXXXXX McAninch at UCSF did my last surgery, removing a little over an inch of scar tissue.

Thanks for query .

Noted your entire history and problems you have been facing since last 50 years.

In a present context bleeding from urethra that you have is mostly due to rupture of a small superficial vein on surface of enlarged prostate which is very common at this age.However the other cause like mass in bladder has to be ruled out .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Endoscopic examination of Bladder (Cystoscopy).

Further treatment will depend upon result of these tests and final diagnosis.

In the mean while you can start taking antibiotic like Cefixime twice daily and combination of Tamsulosin and Dutasteride once daily .

Can a mass in my bladder be confirmed/ruled out without cystoscopy? After 50 years of them, I have great resistance to any more "urethral invasions!"Also, is there an OTC for self-testing for bladder infections?

PS: I have taken Septra DS for the last 2 days and the bleeding has stopped.Are the medications you suggest substitutes for Septra? Since Septra has always been effective for me, is there a persuasive argument for changing to other (three in this case) medications?

Thanks for follow up .

Ultrasound Scanning of abdomen can detect mass in bladder and Enlargement of prostate too.

You
can do dip stick urine analysis at home .It is available in pharmacy.

You need to take Dutasteride and Tamsulosin to reduce the congestion of prostate .That will help to stop bleeding .

You may continue with Sulpha drug as a drug of your choice .
