What Causes Burning Sensation Inside Urethra After Unprotected Oral Sex?

Question: I had an unprotected oral and protected vaginal with a CSW in July 2012 at Korea. After 36 hours I had a mild burn in foreskin which progressed to severe fore skin burning. Colour of the foreskin burning area changed to pinkish. I got tested for STDs. HSV1 came IgG positive and HSV 2 IgG came negative. HIV, VDRL, Chlamydia, Gonereaha all negative. Foreskin burning continued until Sep 2013. After this new symptom started (may be old I don't know) urethra started to itch. Urologist did prostate massage and could not find anything. Till date inside 2mm my urethra burns constantly varying from sharp burn to dull ache all the time. I have taken Harnal D and Toviaz, Antibiotics, Antiviral but no help. Now recently I found after masturbation and when the penis is coming back to normal size the head is unusually swollen and looks odd. This was not there before the encounter. I am worried about any infection in my urethra resulted in trauma of penis. I do. n't want to undergo cystoscopy. I want to know whether my penis is fine. Please suggest ultrasound or other non invasive tests. Doctors in Korea said hsv 1 IgG is very prevalent and 90% of Korean population are positive to hsv 1. Since I am from XXXXXXX please let know whether Hsv 1 is also found in 90% of the population in XXXXXXX also.

HSV 1 IgG positivity indicates that you had a past infection with HSV Type 1. HSV Type 1 commonly causes oro-labial herpes but can also cause genital herpes. However, since you don't mention any history of genital ulcer or erosions suggestive of genital herpes, neither any history of cold sores, therefore most likely yours was a clinically silent infection.

In XXXXXXX 33.3% of individuals are seropositive for HSV-1.

Regarding the burning sensation just inside the urethra, it could be a case of non-gonococcal urethritis.

I would suggest treatment with Doxycycline 100 mg twice daily for a week.
A urinary analgesic like Uriliser syp may be taken 2tsf with a glass of water thrice daily.

Thanks for your reply. I am getting normal erection during masturbation. After masturbation, my penis comes to normal size except the head penis. Only the head of penis remains swollen which looks odd. Please let me know.

Hi.

Glans is a very vascular organ and during masturbation it gets engorged with blood. Therefore it may remain swollen/engorged immediately after the sexual act. It is not an unusual thing and I don't think you have anything to worry about that.

Thanks for your reply, Doctor. I opened my urethra to see where it is burning/itching. With the help of flash light I could see 4 to 5 herpes like small lesions in urethra opening. I showed them to urologist. He said it is normal. But I am having fear this might be herpes lesions in urethra since my blood is positive for HSV 1 IgG. Urethra was swabbed for testing hsv and other STDs. It was negative. Last month my gums were paining. I went to dentist. Dentist said its herpes on the gums. If I have hsv 1 in urethra,does it burn/itch/discomfort constantly? Please let me know if I can resume sex with my wife? I don't want to infect her with herpes or other STD.

If you have any doubt about herpes lesions inside the urethral lining, then the best person to confirm would be a dermatologist.

hsv type 1 IgG positivity does'nt confirm that the urethral lesions are indeed due to herpes because you also have oral herpes. So hsv type 1 IgG positivity could be due to oral herpes.

Anyways it is unlikely that you will have constant burning/discomfort even if it was urethral lesions. The most you can do is to take a therapeutic trial, i.e a course of an antiviral medication e.g Acyclovir 400 mg thrice daily for a week. If that makes you feel fine and you no more see the lesions inside the urethra, it would go in favour of herpes.

I would suggest you to have protected sex with your wife till this doubt is resolved.

Thanks for your reply and advice doctor. Last year only symptom was my foreskIn only burned. Now only urethra tip is burning. Did the infection pass from foreskin to urethra. This is just for information. During pregnancy is it mandatory to check for Hsv 1 IgG? If found positive irrespective of oral, c-section is advised to protect baby?

The lesions can be distributed anywhere along the sensory supply of the affected nerve. Therefore it is not uncommon for the lesions to appear in a different area as long as they are within the sensory supply of the affected nerve.

Antenatal check up involves screening for hsv apart from hiv, vdrl etc

C-section is advised only in case of active genital herpes, during late pregnancy and delivery.

In general, HSV can be transmitted through skin-to-skin contact, but typically only where mucous membranes or cuts in the skin are present.

By exposing a hand to one's genitals with genital herpes, or the mouth with oral herpes, there is little chance of infecting the hand with the herpes virus because the hand itself does not have a mucous membrane. If infection does occur, however, it is often facilitated by small cuts in the skin or torn cuticles, which allow the virus an entrance into the fingers and hand. This condition, called herpetic whitlow, usually causes blisters on the fingers or around the fingernails and is caused by both HSV types.

However, If you touch a sore with your hand and don’t wash your hands immediately after, you run the risk of accidentally rubbing your eye and giving yourself eye herpes.

So keeping your hands clean as much as possible, and avoiding your face when your hands are not clean, is an essential part of keeping herpes under control.
