Question: I am still to get required attention after meeting doctors. My burning sensation is getting worse feet, legs, arms, back, chest waist, stomach, headache that starts from back of head to middle of head. Muscle aches but very reduced. Apart from this i feel okay. Sometimes these symytoms disappear but quickly appear after about a day. Is there any test i should run? What do i do. Is there any doctor in Nigeria you can suggest to me.

Brief Answer:
Serum Vitamin B12 and Vitamin D3 and calcium level to be checked

Detailed Answer:
Hello,

Thanks for writing to us.

Your symptoms suggestive of some nutritional deficiency.
You should go for following tests: Serum Vitamin B12 level, Vitamin D3 level, Calcium level.

If any one or more of them come low in tests, supplementation of the same is needed.

If all comes normal, possibility is somatic disorder. Evaluation and treatment of somatic disorder is done by psychiatrist.

Revert back to me with above test reports.
Also let me know what type of treatment until now you have taken for the same.

I am not aware about doctor in Nigeria. You need to search for your self through internet or ask your GP.

Some of the test results i conducted are out. Calcium is 9.8(8.8-10.2), vitamin B12 is 1554(211-946) vitamin d3 will be out in 10 working days from today. I also conducted a whole lot of other tests every thing seem to be within range but the below mentioned are the ones out of range: alkaline phoshatase is 21(40-129), MCH 25.6(27-32), Neutrophil 29(40-75), lymphocytes 67.4(20-45), LDL 209(100-129) Total cholestrol 285.7(130-230). HDL , VLDL, S. Triglycerides are within range. I am worried about the vitamin b12. What could cause it to go high up and what is the implication?

Brief Answer:
Consult physician and then hematologist as soon as possible with reports

Detailed Answer:
Hello,

Thanks for follow up with reports. It is good that you have mentioned all reports in narration. Images are not so clear.

Three important results in reports are: High serum level of Vitamin B12, high lymphocytes and low neutrophil. There is relative lymphocytosis. This indicates inflammation in body. These are suggestive of some hematological problem (problem related to blood cells). It may be associated with some liver or spleen problem.

Low alkaline phosphates may also relate to some blood cells or liver problem.

Low MCH is related to vitamin B12

High LDL and high cholesterol suggest hyperlipidemia is a risk factor for hypertension, diabetes and heart problems. You need to control your diet in terms of calorie and fat. Regular exercise is also needed.

Considering all investigation there is possibility of liver pathology like hepatitis or any kidney, colon or any blood inflammation or some kind of cancer. I am not at all trying to scare you. But most of the studies suggest that high serum B12 level suggests some blood cancer or may be cancer of liver, colon or lungs related to smoking and alcohol. It is my professional opinion, for exact diagnosis consultation in personal with doctor is must.

Please consult physician as soon as possible with all reports for physical examination. After detail history and examination your doctor can refer you to respective doctor who may be hematologist/onco-physician. If something is there and diagnose early, there will be good treatment response and prognosis.

I wish that there will be nothing serious.

All the best and wish you good health.

Regards,
Dr.Chintan Solanki.

Dr Solanki, thanks for your response and concern. Just want inform you that i also conducted a CRP test and the result is .06(0-5). I thought it is an indication that i do not have inflammation. Also i think Hepatitis should be ruled out as my Hepatitis antigen is non reactive going by the test i performed earlier.
Liver function test and vitamin D3 test results will be out by 20th January.
In the interim is there any any regime i could undertake to bring down the Vitamin B12?
Again is there any address, email or phonei could reach you directly when th test results are out?

Brief Answer:
there is no regime to reduce Vitamin B12

Detailed Answer:
Hi,

Thanks for writing back and more info.

It is good that you do not have inflammation or hepatitis.

There is no specific regime to reduce vitamin B12 level. The best strategy is to treat underlying condition which is responsible for high Serum Vit. B12 level.

So best thing is to consult doctor for finding out the cause.

Hope I have answered your query.
If still any query there, I will be happy to answer.

Nothing swollen.

Brief Answer:
High level of vit. B12 may be indirectly related to your problems

Detailed Answer:
Hi,

High level of vitamin B12 is suggestive of some systemic problem in the body as I described previously.
That systemic problem is causing your current symptoms likely.
Vitamin B12 high level is a finding which indicates to examine and investigate further.
We need to check for that systemic problem.

If all findings come normal, there is possibility of somatic pain disorder.

Hope you got the point which I want to convey.

Thanks & regards,

Dr.Chintan Solanki.
