Urinary tract infection can involve any aspect of the urinary tract. In men this would include the prostate. It is possible to have infection of one or all of the different aspects of the urinary tract- urethra, ureter, bladder, kidneys, prostate
The symptoms of discomfort that you are experiencing are not related to your antibiotic but are part of the infection.
It is possible for an infection involving other aspects of the urinary tract to also involve the prostate, men can also have infection of the prostate without other areas being involved.
Trimetoprim is used to treat some patients with UTI particularly prostatitis , either on its own or in combination with sulfamethoxazole (combination known as bactrim or septra).
Some bacteria are resistant to particular antibiotics but your doctor may have done a urine analysis which showed that the bacteria causing your infection would respond to trimetoprim.
In this case, you should expect to see a continued improvement in the next 24 to 48 hours.
If the bacteria is resistant and your symptoms do not continue to improve then your doctor may consider a repeat urine analysis and a change in antibiotics

Abdominal or back pain are not known side effects of alprim but are symptoms that can occur in some persons with urinary tract infections.
You should take your alprim as prescribed.
Since you seem to be experiencing additional symptoms, you may want to consider calling your doctor in the am regarding your new symptoms to see if he or she thinks a change in antibiotics is needed.
If the urine sample sent to the lab showed the bacteria would respond to alprim, the doctor may not change it at this time.
